How is the Moon phase calculated?

The phase comes from the angle between the Moon and the Sun as seen from the Earth, and the illuminated percentage from their full three-dimensional geometry, calculated from NASA/JPL DE440s orbital data. A principal phase (new, first quarter, full, last quarter) is an exact instant; I name it on the local day it happens and call the days between crescent or gibbous.
